Does cataract surgery hurt?

The idea of eye surgery makes many people feel uneasy. One of the most common questions we hear is: Does cataract surgery hurt?

It’s a fair question. The eye is a delicate part of the body. And for most people, surgery of any kind can be nerve-wracking. But the good news is that cataract surgery is not painful. In fact, most people say they didn’t feel a thing. They are often nervous before the first procedure because of this but when they have the second eye procedure they are much more relaxed as they know it won't hurt.

Let’s explain why.

Cataract surgery is performed under local anaesthetic

You stay awake during the procedure, but the eye is completely numbed. This is usually done with anaesthetic eye drops. In some cases, a small injection around the eye may be used. Either way, you won’t feel pain.

You may be aware of lights, gentle pressure, or movement, but nothing sharp or uncomfortable. Many patients are surprised by how quick and easy the whole thing feels.

What if I’m anxious?

If you’re nervous, you’re not alone. Many people worry about feeling panic during surgery. That’s why a mild sedative can be offered to help you relax. You’ll still be awake and able to follow instructions, but you’ll feel calmer and more at ease.

What happens during the procedure?

The procedure itself takes under 10 minutes. You lie on your back, looking up into a bright light. A small device gently holds your eye open so you don’t have to worry about blinking.

Your surgeon uses a microscope and advanced tools to remove the cloudy lens and replace it with a clear artificial lens. There are no needles near the eye itself. No stitches are needed in most cases.

Will I feel pain after the surgery?

Some people feel a mild scratchy or gritty sensation in the hours after the procedure—like something is in your eye. This usually settles within a day.

Your eye may be a little red or watery for a few days. Over-the-counter pain relief (like paracetamol) is usually more than enough if needed. You’ll be given drops to prevent infection and help your eye heal.

Severe pain is not normal. If you experience strong discomfort, especially with redness or loss of vision, contact your eye doctor right away.

What’s recovery like?

Most people see better within a day or two. You can usually get back to light activity right away—watching TV, reading, or going for a walk.

Within a couple of days, most people return to normal routines, though some precautions are needed (like avoiding swimming goggles and rubbing the eye). Prof Ursell will explain what’s safe and when.
