How Cataract Surgery Works (Step-by-Step)

Cataract surgery removes the cloudy natural lens and replaces it with a clear artificial lens known as an intraocular lens (IOL).

Modern Cataract Surgery Explained

All procedures are performed using the modern technique called phacoemulsification.

A very small incision is made at the edge of the cornea. Ultrasound energy is then used to break up the cloudy lens so it can be gently removed. A folded artificial lens is inserted through the same opening and unfolds into position inside the eye.

Because the incision is so small, it naturally without stitches. This means you will not need to worry about the eye following the procedures. We now do not cover the eye with a patch, no stitches are needed and you can resume normal activities the next day.

What Happens on the Day of Surgery?

Cataract surgery is performed as a day-case procedure.

  • Eye drops are used to dilate and completely numb the eye
  • You remain awake but comfortable throughout
  • The procedure typically takes less than 10 minutes
  • You return home the same day

Is Cataract Surgery Painful?

No, cataract surgery is not usually painful. Most patients only feel a little mild pressure or awareness of light during the procedure.

A cataract doesn’t have to be removed just because it’s there. Surgery is usually recommended once it starts interfering with everyday activities like driving, reading or watching television.

Cataracts don’t need to be “ripe” before removal, in fact, surgery tends to be safer and more effective when performed at an earlier stage.

Your individual circumstances will be discussed at consultation, and surgery is only carried out when you need it.

Cataract surgery is a day case procedure using a technique called phacoemulsification. A small, self-sealing incision is made at the edge of the cornea, the cloudy natural lens is gently removed, and a folded artificial lens is inserted and unfolded into place.

Most patients are awake throughout, with eye drops used to numb and dilate the eye rather than an injection, and the procedure itself typically takes under 20 minutes per eye.

Cataract surgery is generally very safe, but as with any surgical procedure, results can’t be guaranteed and complications occasionally occur. The chance of a serious complication that severely affects vision is around 1 in 500.

  • Mild discomfort: some pain, light sensitivity and glare are common in the first few days. Severe pain may indicate raised pressure or infection, so contact us immediately if this happens.
  • Adjusting to light: as the cloudy lens is replaced with a clear one, it can take time to adjust to more light entering the eye. Colours often appear brighter as a result.
  • Glasses after surgery: the implant lens strength is calculated to minimise the need for glasses, and most monofocal patients see clearly at distance with simple reading glasses for close work. Some patients still need glasses for distance, reading or both despite successful surgery, in rare cases the implant may need to be exchanged.
  • Stitches: modern keyhole surgery doesn’t require stitches. On the rare occasion one is used, it may cause temporary astigmatism or blurred vision until removed around two weeks later.
